MJF Rehabilitation, PA accepts credit/debit, cash or personal check for payment of co-pays, percent payments as dictated by your insurance provider, and other fees that may arise.  We strive to inform patient’s ahead of time regarding the nature of any fees incurred, however, please understand that not all fees can be absolutely estimated.  We do request payment upon your visit, and appreciate your understanding.  Please feel free to ask us if you have questions.

Yes. We do ask that you obtain a referral from your primary care doctor in order that all information regarding your medical history may be obtained in efforts to improve the quality of your care. As specialists, we want to utilize your appointment time to delve into the specifics of the ailment for which you are seeing us, rather than spending significant time collecting other medical history.  Obtaining such information in advance allows us to be better prepared to safely and optimally meet your treatment needs, as well as to ensure insurance information, which we can all agree is often confusing, is fully understood and can be appropriately utilized at your appointment time without delay. We appreciate your understanding.
